Yes. As a translator I once found a major mistake in a financial text while translating the annual report, which is sent to all the shareholders. I got in touch with the corporation's finance dept. and they quickly corrected it. This was part of my responsibility as I was the last link of the chain. You realize though that you do not do it on your own without consulting with the author.

It depends, usually a translator and the client stay in touch during the translation process in case something is unclear. It is up to the client; in most cases they just want a correct translation, so without possible mistakes from the source language. If the translation has to be as accurate as possible (for legal reasons for example), they should do that. It's like in any other job: the customer is always right!

You are a translator not the writer/speaker. You are only hired for translation. You should only translate, and not anything else.

It’s kind of like, you are a waitress and the food doesn’t look like it taste good, you put some sauce into it even though the cook or the customer didn’t ask you to.

yeah, because they would make themsleves sound like a fool. Its part of their job, but if the speaker doesnt want them to alter it they wont.
